1. Person and jurisdiction
2. Advance care wishes (living-will preparation)
Record preferences to discuss with clinicians and your jurisdiction's official advance-directive forms. This section is not itself a valid medical directive.
3. Estate planning notes (last-will preparation)
4. Assets and liabilities inventory
Use estimated current values, dates and descriptive identifiers only. Asset lists help with planning but do not automatically transfer title or override beneficiary designations, trusts, or applicable law. Avoid double-counting property and associated loans.

This inventory is separate from Thesara's savings/debt snapshot; do not interpret it as a complete or legally appraised estate value.
5. Intended asset distribution worksheet
Optional planning notes identifying who you would like to receive specific assets. These entries are not a will, a beneficiary designation, a transfer of title, or binding instructions. Review jointly owned property, trusts, beneficiary designations and applicable jurisdictional rules with qualified counsel.
Firmament legal-information pathway
After preparing your private worksheet, consult Firmament for legal research and current official resources. Estate documents and health-care directives are separate legal instruments and must meet the requirements of the appropriate jurisdiction.
Estate & elder-care resource pathway
Explore Firmament's connected senior support, family resources, insurance education, legal research and estate-planning preparation guide. This link does not transfer your worksheet data.
6. Follow-up checklist
Review applicable local requirements for a last will, advance health-care directive, health-care proxy, durable power of attorney, and any necessary witnesses or notarization. Confirm asset titles, beneficiary designations, insurance policies, and where originals will be stored. Review after major life changes.
This printout is an unsigned planning worksheet. It is not a substitute for legally executed estate-planning or advance-directive documents.
